MIDWEST FUTURE PROSPECTS PLAYER/PARENT CONTRACT
Midwest Future Prospects and the undersigned Player/Parent/Guardian hereby enter into the following agreement:
I, _____________ __________ (Parent/Guardian of player if a minor have been informed of the rules of play concerning the upcoming 2026 season. I further understand that by signing this agreement I am entering into a COMPETITIVE baseball program, with no guarantees made by Midwest Future Prospects with respect to playing time or playing position on the team.
MIDWEST FUTURE PROSPECTS RULES:
1. No Tobacco Products, Alcohol or Drugs
No Midwest Future Prospects player shall have in his possession, or use, tobacco products, in any form, alcohol or illicit drugs at any time or place during the regular season or any tournament. Violation of this rule shall result in immediate disqualification for the remainder of the season and tournaments for the year in question. If a violation of this rule occurs while a Midwest Future Prospects team is on a road trip, the player’s manager or coach shall have the authority to immediately send the player home.
2. Conduct
All Midwest Future Prospects players shall follow all rules of the baseball organizations that Midwest Future Prospects is currently participating in ( USSSA, CABA, AABC ), and their codes of conduct, and will promote the best interests of these programs at all times. No abusive or profane language or conduct directed at opposing players, managers, coaches or fans is allowed at any time. Players will not argue with umpires or throw bats, helmets or other equipment. Players will stay on the field or in the dugout or bull pen area during games. There will be no eating during games.
3. Team Fees and Penalties
I understand that if I voluntarily quit for any reason or I have been removed from the team due to a violation of the rules, I will not be eligible to play again for Midwest Future Prospects at any level. I further understand that I will forfeit any payments already made and will pay the balance of the agreed team fees as stated in the offer letter within (7) days of the written notice. Player fees are non refundable regardless of injury, termination, or if you quit the team.

WAIVER AND RELEASE OF LIABILITY FORM
I acknowledge I have voluntarily chosen to participate in a Midwest Future Prospects baseball or softball program and I have full knowledge of the risks this activity presents, including travel to, participation in, and returning from the activity. I am aware portions of this activity are not guided or supervised by the Midwest Future Prospects staff.
By taking on a role as a participant, in order to participate and/or use the practice facility, I agree to assume any and all risk of injury and death. I further understand and agree to assume responsibility for risk of theft, loss or damage to my personal property which may occur at any time throughout my participation with this program.
I understand and agree as a condition of participation in this activity, I will release from liability and will indemnify Midwest Future Prospects, their officers, directors, agents, coaches, assigns, successors, lessors, and owner for any damage, injury, or death to myself or any other persons or property, in any way connected with my participation in this activity. I understand there may be specific hazards associated with this activity to include injury and/or death, and I accept full responsibility for these hazards.
I have carefully read this agreement and fully understand all of its terms and conditions. I understand this is a release of liability, which could legally prevent me from filing a lawsuit or making any other legal claim for damages in the event of my death or injury. With this knowledge, I am entering into this agreement fully and voluntarily. I agree the agreement is binding upon me, my spouse, my heirs, my children including any guardian for the children, my assignees, and legal representatives.
I understand and agree by signing this waiver and release on behalf of my minor child that I am giving up the same rights for the minor as I would be giving up if I signed this document on my own behalf.
MEDICAL TREATMENT CONSENT:
I, the person and/or legal guardian of the name listed, authorize The Midwest Future Prospects staff to seek medical treatment for the listed named, as they may deem necessary at any medical facility. I consent to any x-ray, anesthetic, medical, or surgical diagnosis or treatment and hospital care subsequently deemed necessary by a licensed healthcare provider. I understand this authorization is given in advance of any specific diagnosis, treatment, or hospital care and it is given to provide Midwest Future Prospects staff authority to seek medical treatment, and to provide a licensed health care provider the authority to administer this treatment as he/she judges necessary to the name listed. I accept responsibility for the payment of all services rendered. I understand whenever possible, Midwest Future Prospects staff will make a good faith effort to contact me before treatment is sought.
